Filing a Complaint

Ontario (ON)

If you are located in Ontario, you can use the Ontario Soccer Match Official Incident Forms to file a report.

Assault Pre-Report and Referee Assault Forms must be submitted to Ontario Soccer, League and District within 48 hours of the game being reported on.

British Columbia (BC)

If you are located in BC, you can use the BC Soccer Discipline Reporting Forms to file a report.

On this website, you can also log in to the Match-related Discipline System where you can access live status updates of cases you have been affected by.

Saskatchewan (SK)

If you are located in SK, you can report referee abuse by following the steps on the Saskatchewan Soccer Association website.
